Here’s how you can start taking advantage of your membership (more steps below):

Download our Communication App
Click here to download the Patient Portal. The portal will give you access to schedule an appointment, message us securely, as well as access to your health records. The patient portal will also be the platform we use for any telehealth visits. Now you are ready to connect with us at your convenience! You may also call the practice at 573-995-2213.
STEP 2
Medical History Form
Once you schedule your first appointment, you will receive an email requesting you to complete your medical history form electronically prior to your visit.
STEP 3
Past Medical Records
If you would like us to obtain your past medical records, please complete this form and send it in to our office. We will work with your previous physician’s office to obtain your records.
